Mental Health and Dementia Nurse/Practitioner

About the Role

What you'll be responsible for: Act as a specialist practitioner, working with a caseload of people referred to the

mental health and dementia service. Be responsible for the assessment, planning, implementation, and evaluation of care

Adhere to all Trust policies, procedures, and guidelines To work collaboratively with other services to advise and

ensure proactive support to people with dementia/mental health in the Trust. Providing frontline support for patients

and staff Experience Strong organisational skills and the ability to analyse situations and be able to make the right

decisions. Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with
