About the Role

Key Responsibilities: Undertake holistic assessments of patients health needs using an evidence-based framework and be

able to assess and use sound clinical knowledge. Develop, implement, and evaluate individualized care plans in

collaboration with patients and the multidisciplinary team. Monitor therapeutic response to prescribed medications and

monitor their effectiveness, adhering to prescribed protocols. Mental Health Assessments: Clinicians should have

experience in completing mental health assessments and delivering evidence-based interventions in community settings

Provide support and education to patients and their families regarding mental health conditions and treatment plans.
