About the Role

Key Responsibilities Clinical Care Provide comprehensive mental health assessments, formulation and care planning for

patients presenting in primary care Deliver evidence-based interventions for common mental health conditions (e.g.

anxiety, depression, PTSD, severe mental illness) Undertake risk assessments, crisis management and safeguarding

interventions Manage a defined caseload and work autonomously with supervision as required Liaise with GPs,

multidisciplinary teams, and secondary mental health services to ensure coordinated care Support long-term condition

management for patients with serious mental illness Leadership and Service Development Provide clinical supervision,
