Mental Health Practitioner - Homeless Mental Health Team

About the Role

Fixed term post To join the integrated MH and physical team supporting the homeless in the county of Nottinghamshire.

This is a newly established team and we have new funding to increase staff capacity. We are looking for an individual

passionate about working in the sector and in return we offer training and support to enable working in this specialist

team. We are looking for a Mental Health practitioner which includes Nurses, AHP's and Social workers. Daily duties

include managing a caseload of service users in the community experiencing homelessness. Triage of referrals, provision

of initial assessments and one-to-one interventions with service users. To undertake and record comprehensive need/risk
