About the Role

We are looking for an enthusiastic mental health practitioner to join our small friendly, nurse led team.

You will require experience of working in a mental health setting and awareness of delivery of community mental health

care.

You will hold and manage a caseload of people (for up to 6 months) facilitating assessment, care planning, sign posting,

short term interventions and safe discharge. Your role will also include liaison with other multi agency services.
