The  Prevention & Wellbeing, Adult Mental Health Team is an award-winning  Adult Social Care Provider  in Cheshire West

and Chester .

We work alongside a wide range of consultants, clinical experts and specialised agencies providing rehabilitation,

treatment programmes, care planning, assessment, and long-term focus on recovery and overall wellbeing.

We have four core support offers, all with a view to offer the tools and guidance needed to navigate life’s choices and

personal pathways.
