appropriate format, listening to their needs and signposting and referring to resources as required, in a time efficient
way. Be a single point of access regarding information and advice for those children and young people affected by
neuromuscular conditions, their families/carers and the professionals working with them. Work alongside the clinical
nurse specialists/AHPs to provide effective co-ordination of hospital and community care for children and young people
