About the Role

The post holder is responsible for leading the Neonatal Community Outreach Team, the assessment of care needs, the

development, implementation and evaluation of programmes of care without supervision in both the hospital and the

community setting. The post holder will work across site in both hospitals and Community environments when required.

The post holder is required to teach and supervise families and other nursing and non-nursing staff and be a resource

person within both units for discharge planning and outreach support.

To be a professional role model, leading and co-ordinating the continuation of care in the community/home stay of babies
