JOB OVERVIEW
To provide an expert level of professional practice within the Neonatal Community Outreach Team, to babies and their
families as partners in care before and after discharge from the Neonatal Unit/Transitional Care, using a flexible
patient centred care approach.
MAIN DUTIES OF THE JOB
You will be an expert in the assessment, planning, implementation and evaluation of a defined caseload of infants
