About the Role
Undertake comprehensive initial and review Children & Young Peoples Continuing Care (CYPCC) Assessments in line with the
National Framework (2016) Support CYPCC eligible individuals and or their representatives working collaboratively with
families, carers and multi-disciplinary teams to design and implement personalised care packages. Monitor and review
care packages to ensure they remain responsive to changing needs. Contribute to service development, quality improvement
and safeguarding initiatives across Gloucestershire. Support the development and review of CYPCC Personal Health Budgets
(PHB) in line with the CYPCC National Framework (2016). Ensure accurate and open communication and co-ordination with a
