Nurse Associate - Enhanced Primary Care Mental Health - Dementia Focus

About the Role

To deliver structured physical health checks, clinical observations, and monitoring for individuals whose mental health

needs place them at risk of physical health deterioration or unmet health need. The post holder will support early

identification of risk factors, contribute to long-term condition monitoring, and work within a multidisciplinary

primary care mental health model to ensure safe, equitable and holistic care. The role supports people including but not

limited to those living with: Dementia Serious Mental Illness (SMI) Neurodivergence Gender dysphoria Other mental health

conditions associated with increased physical health risk
