About the Role
An exciting opportunity has arisen to join the Mid Specialist Dementia and Frailty pathway team within Mid Essex. We are
looking to recruit a motivated, hardworking team leader who has excellent communication skills with a passion for
driving service improvement and working collaboratively with other health care providers and third sector agencies.
The successful candidate will be responsible for leading the Chelmsford team, ensuring the delivery of a high level
service providing a proactive assessment, diagnostic, monitoring and management of patients with dementia of any age and
people over the age of 75 years with complex Mental Health needs who have an age related frailty.
