About the Role

The Adult Community Nursing service Single Point of Access (SPA) team is looking for a Nurse Practitioner to join its

Clinical Assessment Team (CAT). This post is ideal for nurses who are enthusiastic, responsible and committed to the

highest standard of care.

The CAT Team is situated within the Adult Community Nursing Service at Homerton Healthcare and works alongside District

Nursing Neighbourhood Teams and Specialist Services including Tissue Viability and Continence nursing teams. The Team

works across City and Hackney and undertakes holistic care assessments and implementation of care in patients’ homes,
