About the Role
Assess patients within a Crisis situation and offer appropriate nursing assessment intervention, and review to meet
their needs using advanced clinical assessment skills Assesses, plan, implement and reviews routine through to complex
nursing care plans using research and evidence based knowledge to underpin practice and which support avoiding a
hospital admission. To supervise, lead, inform and delegate staff as appropriate to the needs of the service by
utilising skills and resources effectively as directed by the Urgent Community Response team leader. To be an expert
resource within the team regarding all aspects of nursing care. Demonstrates a high level of intuitive and creative
