You will be providing nursing care for palliative and end of life patients, domiciliary and ambulatory wound care,
catheters, leg ulcer care and minor injuries. As a band 6 Community Nurse Specialist you will work clinically and
provide a day to day leadership to the team. The role involves autonomous, lone working, using specialist assessment
skills to plan, implement and evaluate programmes of care for individual patients. You will work as part of a team,
liaising with other professionals such as specialist hospice nurse, social workers, GP, Podiatry and Tissue Viability.
