About the Role

Clinical responsibilities - To facilitate discharge of Neonates providing the follow up support in the community -

Initiate early discharge if appropriate to the patient - Collaborate & build relationships with Multidisciplinary teams

- Act as a source of speciality expertise to parents& carers, Neonatal outreach and the Neonatal teams. - Provide all

education teaching/training to parents & carers to facilitate discharges. - Participate In planning and implementation

of quality initiatives and targets, in conjunction with the MDT team. - Drive and hold a current driving licence

Managerial responsibilities - Lead and manage the outreach service, analysing and scoping to continually develop the
