JOB DESCRIPTION
The jobholder will be responsible for the management and performance of the nursing staff in the Hospital at Home
Service. Promotion of the Hospital at Home Service and Geriatric outpatient services. Monitoring the provision of care
provided by the Hospital at Home Service. Measuring the outcome of patient care. Maintaining effective communication
