About the Role

We take referrals for medical and/or social crisis from the community and (e.g NWAS, GPS ) and SRFT departments, aiming

to promote early discharge from hospital and avoid unnecessary hospital admissions; by implementing clinical

treatment/care plans in the patient's own homes in the Salford area.

The team manages acutely unwell patients at home using a variety of pathways e.g. IV antibiotics via cellulitis pathway

, DVT pathway we are currently developing further pathways to manage patients in their own environment and prevent

unnecessary admissions to hospital e.g. heart failure pathway.
