Social Care Trusts/Somerton House (this role will be predominantly based in the community up to 25 miles radius from

home) Role To provide support and respite care for patients and families in their own home. To deliver high quality

nursing care to patients with palliative care needs by implementing and evaluating individualised care plans. The
