About the Role

As a Therapist within the Home First Team you will be involved in many aspects of a patient’s journey from hospital to

home. Our team support patients: within the Emergency Village (EV) where they need additional support to return home;

through the triage of patient referrals referral to the service with MDT discussion around what pathway is suited to

meet their needs and promote independence; and in patient’s homes or short-term placements to help them achieve their

rehabilitation goals. This role is primarily based within the community, however there is flexibility in the location of

assessments as the service continues to develop, so patients may be assessed for home in the hospital, at home, or
