About the Role
This role delivers a nursing service that provides high quality care to patients and meets clinical and regulatory
requirements. You will assess plan and deliver high quality patient care and support patients and carers to understand
their condition medication and lifestyle needs so they can manage their own health with confidence. You will work with
internal and external colleagues and ensure all care is safe effective and compliant. Key accountabilities Assess plan
deliver and evaluate high quality care for a caseload of patients with varying conditions and needs Carry out
comprehensive assessments and create individualised care plans updating these as the patient condition changes
