guidelines as a named nurse (or key worker) for a defined group of patients and take responsibility for: protecting and

supporting the health of individual patients and clients the assessment of care and health education needs the

development, implementation and evaluation of programmes of care including discharge planning for each patient 2. To be

designated to take charge of the ward/department regularly in the absence of the person holding continuing
