About the Role

DUTIES AND RESPONSIBILITIES: To promote high quality, pesonalised palliative and end of life care across a range of

health and social care settings by identifying and developing the skills and knowledge of multi-professional staff in a

variety of health and social care settings to ensure best practice and high-quality services to all patients with a life

limiting illness. To recognise individual, organisational and system wide issues which impact upon the provision of

palliative and end of life care, tailoring facilitation of learning to take account of and address such issues. Lead on

the development of proposals and innovative educational initiatives in line with identified gaps in provision and
