About the Role
The post holder will be a proactive role model who designs, plans and delivers education and training to enhance the
end-of-life care provided by professionals across Calderdale and Huddersfield Foundation Trust (CHFT) and Calderdale
community settings including GPs and Care homes.The hours will be between Monday and Friday, working flexibly to cover
the needs of the service.
They will work alongside the CHFT End-of-Life Care (EOLC) Facilitator in determining the Palliative and EOLC training
and education requirements. The post holder will establish, develop and maintain effective working relationships within
