CNWL are pleased to announce a vacancy within: Hillingdon Single Point of Access (SPA) and YLL 24/7 with an opportunity

of 6 month rotation programme in the Specialist Palliative Care Team

We are looking for an innovative, enthusiastic and dynamic nurse to join our team whose aim is to:

• Improve the coordination and continuity of care and support for patients and their families/carers.

• Enable responsive in and out-of-hours nursing care for people in the community who are at the end of life

• Work within the SPA, to enable direct access to services and support for health and social care professionals,
