Patient flow co ordinator

About the Role

Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other

members of the MDT; doctors, Nurses, occupational therapists, physiotherapists and other hospital assessment and

discharge team members. Establish and maintain productive integrated working relationships with social service and

community healthcare colleagues as required. To develop strong working relationships with ward teams, Discharge Lounge,

Therapy teams, Discharge co-coordinators pharmacy etc., to support and co-ordinate timely and appropriate assessment and

safe transfer or discharge of people from the emergency department. For full list of duties please see job description
