This post will be based within the (Fife-wide) Integrated Discharge Team, mainly based within Victoria Hospital,

Kirkcaldy and on occasion may be asked to cover Community Hospitals across Fife. This post involves comprehensive

assessment and discharge planning.

We work to support discharge to meet PDD. Our ethos is patient centred pathways within the Home First Model. The post
