About the Role
Key Responsibilities Patient Admissions and Discharge Take the lead in ensuring safe and timely admission and discharge
of patients, actively identifying and addressing any delays in the discharge process. Communication and Advocacy: Act as
an advocate for patients, carers, and relatives to enhance their admission and discharge experience. Facilitate
sensitive discussions regarding admission and discharge plans, addressing any disagreements and ensuring all parties are
informed and aligned Multidisciplinary Collaboration Chair multidisciplinary admission and discharge meetings, ensuring
that all relevant stakeholders are engaged in the discharge planning process. Patient-Centred admission and discharge
