About the Role
We wish to recruit a nurse who has a clear understanding of developing the role of a nurse who can assess patients
within both a community and hospital setting. We work predominantly but not exclusively with older people. Our focus is
ensuring patients health needs, often arisen from multiple co-morbidities and frailty, are met with the appropriate
level of care. We work alongside the MDT to gain the best patient outcomes and facilitate patients to their preferred
place of care, avoiding delay where possible. Candidates should demonstrate the ability to work in a team which delivers
innovative, responsive services to meet both the needs of patients and the organisation. We are committed to staff
