Who we are looking for A Registered Nurse with General Practice experience Experience in chronic disease management and
diploma qualifications in chronic disease management including asthma, COPD, diabetes, heart failure, cardiovascular
disease An independent worker who is able to deliver projects within oversight supervision Excellent personal skills
