PCN Manager and Transformation Lead
About the Role
The post will evolve as services develop, but the postholder will be required to undertake the following duties General
Act as lead for the Network Contract DES and encourage all practices in the network to engage in the process, ensuring
strong performance against the DES. Work collaboratively with practice managers and clinicians to develop the PCN
workforce, including workforce planning, recruitment, deployment, mentoring, and appraising. Maintain confidentiality at
all times. Support PCN Clinical Directors to develop and deliver PCN plans and performance requirements. Develop quality
monitoring and support mechanisms for staff working in subcontracts or under the Additional Roles Reimbursement Scheme
