About the Role

Job Summary Medway Rainham Primary Care Network (PCN) is seeking a skilled and motivated PCN Nurseto deliver proactive,

preventative and time-limited nursing care across the network. Working across GP practices and through home visiting,

the role focuses on early identification of clinical deterioration, admission avoidance and supporting patients to

remain well, independent and safely managed. The post-holder will manage a defined PCN population caseload, using risk

stratification and population health approaches to identify patients who would benefit from early intervention and

coordinated care. This role supports patients before they require long-term or intensive District Nursing input and does
