PCN Nursing Associate

About the Role

Key responsibilities & Duties Work within the PCN to provide and monitor care, alongside the wider PCN team.

contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect

supervision. Improve safety and quality of care at every opportunity. Undertake additional clinical training as required

by the PCN to support the Network Contract/DES Services Specification. Support Advanced Practitioners, Paramedics,

Clinical Pharmacists and the wider team to enable them to focus on the more complex clinical care. To support with

chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN
