About the Role

Clinical Responsibilities Triage, assess and treat patients at home, or in the surgery when directed to do so. Make any

necessary referrals in collaboration with the referring or duty clinician to other members of the primary health care

team, community or hospital teams or social care team as appropriate. Assess, diagnose, plan, implement and evaluate

treatment, interventions and care for patients presenting with an undifferentiated diagnosis on an acute basis within

the boundaries of competence, seeking the advice of the referring or duty clinician if outside level of knowledge and

scope of practice. Where appropriate, discuss with the referring or duty clinician doctor the needs of patients
