PCN Paramedic

About the Role

Working closely with Primary Care Network Clinical Directors and take direction from them as to the needs of the

service. To support the PCN GP practices with home visiting workload. To work as an autonomous practitioner working

independently and with other health care professionals to assess, diagnose and treat patients with a range of acute,

non-acute and chronic medical conditions, initiating treatment as appropriate. To visit patients either in their own

home or in a care home environment. To proactively manage housebound / care home patients to increase the quality of

care provided to this vulnerable group of patients. To assess, diagnose and treat undiagnosed and diagnosed conditions
