About the Role

The PCN Pharmacist collaborates with GPs to resolve medication issues and enhance patient care. This role involves managing long-term conditions, ensuring adherence to high-risk medications, and conducting clinical audits.

Responsibilities

• Resolve day-to-day medication issues with GPs.
• Manage long-term conditions in collaboration with practice nurses.
• Ensure compliance with monitoring for high-risk drugs.
• Conduct clinical audits and provide education to healthcare professionals.
• Support the practice team in medicines optimization initiatives.

Requirements

• Registered pharmacist with relevant clinical experience.
• Strong knowledge of medication management and optimization.
• Excellent communication and teamwork skills.
• Ability to conduct clinical audits and lead quality improvement.
• Experience in primary care settings preferred.
