Dream Medical is working with a Health Care Centre in the Yeovil area who urgently require part time Practice Nurse
An opportunity has arisen for a Practice Nurse to work across two sites in the Yeovil area. You will part of the nursing
team responsible for ensuring the practice provides optimum care relevant to the practice population, in the practice
and home environment. You will focus on supporting patients to be healthy, monitoring long term conditions, health
prevention and screening activities. The ideal candidate will need to demonstrate relevant experience in Chronic Disease
