About the Role
Great West Road PCN is seeking a motivated and clinically focused Clinical Pharmacist to join our multidisciplinary
primary care team.
West London NHS Trust will host the role. However, it is entirely GP practice based, and the successful applicants will
have the opportunity to embed the role of the PCN clinical pharmacist within the PCNs.
This role is delivered in line with the NHS 2025/26 Network Contract DES and focuses on providing high-quality clinical
pharmacy services across the PCN.
