About the Role
Key responsibilities include: Primary Care: Conduct structured medication reviews and medicines reconciliation at GP
practices, during clinics, or following patient discharge from other care settings. Support long-term condition
management (e.g. hypertension, cholesterol, diabetes, asthma). Provide advice and resolve medication queries by
collaborating with GPs, nurses, and other prescribers. Contribute to quality improvement projects, audits (QOF/IIF), and
prescription safety initiatives. Update and maintain accurate electronic prescribing and medication records (e.g.,EMIS,
SystemOne).Community Pharmacy: Deliver NHS and private pharmacy services, including dispensing, clinical checking, and
