About the Role

START is a community mental health team located within The South London and Maudsley NHS Foundation Trust working with

homeless people across Southwark, Lambeth, Croydon and Lewisham. The teams offer a holistic service for people with

triple co-morbidity of need across mental health, substance use and physical ill health.

Key Responsibilities:

The post holder will:

• provide physical health assessment, interventions and referrals onwards to clients with acute and chronic physical ill
