About the Role

Our neighbourhood team consists of physiotherapists and physiotherapy assistants, community nurses, occupational

therapists, healthcare assistants working alongside the Worcester GPs to support peopleliving in their own homes.This

includes admission prevention and pathway one assessments as well as supporting urgent community response.The successful

candidate will have the opportunity to work with a varied caseload which involves holistic falls assessment and

treatment, orthopaedic, neurological, frail elderly, respiratory, musculoskeletal, palliative and end of life, urgent

community need eg equipment, sudden deterioration, manual handling difficulties, and carer breakdown. For full details
