Practice Based Care Co-ordinator

About the Role

The Care Coordinator will play an essential part in supporting our clinical team by helping patients navigate their

care, coordinating health reviews, and ensuring key patient groups are proactively supported. You will be the first

point of contact for several patient pathways and will work closely with GPs, nurses, administrative staff, and external

healthcare providers
