About the Role
Primary Responsibilities: The following are the core responsibilities of the Practice Nurse. There may be occasions
where other duties are required, depending on workload and staffing levels. Clinical Care Assess, plan, and deliver
evidence-based nursing care to meet patient needs. Manage and support patients with chronic conditions such as asthma,
COPD, and diabetes, providing patient-centred reviews and care plans. Identify and support patients at risk of
developing long-term conditions to help prevent deterioration. Provide routine nursing care, including cervical
screening, in line with NICE guidance and national frameworks. Deliver wound care, compression bandaging and Doppler
