About the Role

Deliver and evaluate a wide range of health interventions to meet patients health and social care needs. Promote health

and well-being through individualised, person-centred care plans, especially for patients with or at risk of long-term

conditions. Manage and treat long-term conditions (e.g., asthma, COPD, diabetes, hypertension) in line with national and

local policies, including running dedicated clinics and reviews. Conduct NHS health checks for new patient registrations

and provide general and specific health screenings and assessments. Review medication for therapeutic effectiveness

using evidence-based practice and national protocols, supporting patient adherence to treatments. Triage and prioritise
