Practice Nurse Chronic Disease

About the Role

The successful candidate will be professional, motivated and able to work collaboratively as part of our friendly

Practice multidisciplinary team. On appointment you will ensure nursing services are delivered effectively to the

entitled patient population. The successful candidate will, dependant on skills and experience, take a share of the

nursing responsibilities within the Practice which may include areas such support in infection prevention and control,

health promotion, chronic disease management, health prevention, as well as supporting the practice management team in

the reviewing and delivery of clinical policy and procedure.
