About the Role
KEY RESPONSIBILITIES Clinical Practice: To assess, design, implement and evaluate safe, creative and effective treatment
plans for wound care in collaboration with the patients diverse needs, their carers and other professionals. To take an
holistic approach to the assessment of the patients with wounds to include consideration of all the physical,
psychological and social factors that affect wound healing. To support the practices with managing patients living with
long-term conditions, including frailty, and provide relevant care as necessary. To undertake clinical tasks such as
vital signs, ECG, phlebotomy, diabetic foot checks, sample processing Responsible for the safe monitoring, management
