Practice Nurse - Learning Disability

About the Role

Clinical To undertake annual health checks for all patients who have a learning disability across gtd primary care

practices including a physical examination when required. To provide the service user with an annual health action plan.

To support the service user to navigate services within primary care. To have an understanding of Quality Outcome

Framework and use this to support annual health assessments. To act as a resource and point of contact providing advice,

guidance and support for all gtd healthcare staff including trainee nursing associates when providing care for

individuals with a learning disability. To facilitate the meeting of individual health care needs for people with
