Practice Nurse - Whitworth Medical Centre

About the Role

The post holder will be responsible for the delivery of General Practice Nursing to the practice population. The focus

of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the

provision of preventative health care. As an autonomous Practitioner, the Practice Nurse is responsible for the care

delivered to patients, demonstrating critical thinking and skills in clinical decision-making in the management of

Patients. They will work collaboratively with the General Practitioner and other Practice Nurses to support the delivery

of a healthier Whitworth through listening and responding to Patients needs. The post holder may, in the future, be
