About the Role
Job Purpose The Practice Nurse will work as part of the multidisciplinary team within the GP surgery, delivering
high-quality, evidence-based nursing care to patients. The post holder will provide general practice nursing services
with a particular focus on the management of long-term conditions (LTCs) such as diabetes, asthma, COPD, and
cardiovascular disease. They will support health promotion, disease prevention, and proactive patient care within the
