About the Role

Clinical Nursing Practice and Care Management Manage the nursing care for individuals and groups within the practice

population, including direct access to specialist nursing care for patients within the practice setting. Liaise as

appropriate with specialist nurses. Undertake diagnostic, health surveillance, health screening, and therapeutic

interventions within a broader health promotion/public health strategy which includes responsibility for health

promotion information in the practice. Manage programmes of care for patients with acute and chronic disease by

planning, providing and evaluating care including audit, recall and evidence based practice. Work with and participate
