About the Role

Communication To work closely and collaboratively with other members of the practice management team to achieve high

quality, safe and evidence-based patient care consistently across all sites of Tower Family Healthcare To work

collaboratively with both clinical and non-clinical staff to deliver seamless care to the patients of Tower Family

Healthcare. To work collaboratively with the ICBs Medicines Optimisation Team to support the implementation of medicine

related ICB initiatives within the practice. To liaise closely with local community pharmacists, community nurses, care

home staff, hospital pharmacists and other care providers, as necessary to ensure accurate exchange of
